1. Introduction: These terms and conditions apply to the WSD Wellness Plan between you, the customer or patient, and WSD in our capacity as a health service provider. Your visits to any WSD practice and the services we provide are also subject to our standard terms and conditions.

2. Privacy and Use of Personal Information: All personal information is handled according to our Privacy Policy. Your use of the WSD website is governed by our Website Terms of Service.

3. Plan Overview: The WSD Wellness Plan is a 12-month subscription service providing specified treatments and discounts at participating WSD practices. The plan automatically renews for another 12-month period unless you cancel or notify us of your decision not to renew before the current term ends.

4. Benefits: During the 12-month term, the plan offers one standard examination and X-rays, two 45-minute hygiene appointments, and 10% off additional treatments, excluding dental implants, orthodontic services, oral medicine services, sedation in all forms, and specialist care. If paying monthly, the first clean session and or dental check-up is available after enough credit has accumulated on the plan.

4.2 Please note: The plan may not be suitable for all patients. Your dentist can advise on the best treatment plan if this plan is not suitable for you. If advanced or additional hygiene care is needed, an extra fee will be charged and you will be informed of this before treatment is completed. It is your responsibility to use the benefits before the end of the 12-month term.

5. Registration and Patient Portal Use: When purchasing the plan, you must provide a valid email address. WSD uses this to give you access to the patient portal and to communicate updates about your plan, including renewal notifications and changes. You must inform us in writing if your contact details change.

6. Participating Practices Only: The plan is valid only at participating WSD practices and must be used at the participating practice where it was purchased. It cannot be used at non-participating practices, including those specialising in oral surgery, endodontics, or orthodontics. If purchased online, it is valid only at the selected practice.

7. Term and Renewal: The plan and its benefits are valid for 12 months from the signup date. Upon registration, you commit to the full 12-month term. If you cancel before the end of the term, no refund is given and any outstanding payments remain due. The plan automatically renews unless you notify WSD by email before the renewal date. WSD will send a reminder approximately 28 days before renewal and a confirmation email on renewal.

8. Pricing: You will be informed of the total amount payable for the 12-month term before registration. WSD reserves the right to change plan pricing at any time, with changes applying at renewal. Existing customers will be informed of any price changes in the renewal email. Some patients may purchase the plan retrospectively within 24 hours of an initial hygiene or exam and X-ray appointment, at the discretion of the practice.

9. Payment: The plan is a 12-month subscription paid by direct debit from your credit or debit card. Payments can be set up monthly or annually in advance and cannot be changed later. The plan cannot be paid using Q-card finance offers, Afterpay, or other financing methods unless specified. You authorise WSD to deduct payments automatically. If payment fails, WSD will retry up to three times. If payment still fails, WSD may contact you for alternative payment or cancel the plan. Stripe New Zealand Limited is the payment gateway used by WSD.

10. Cancellation: You can cancel your plan at any time by providing written notice to your participating practice. If you cancel within seven days of registering or renewing, you will be charged for all benefits used before cancellation at the full retail price of the participating practice. If you cancel after the cooling-off period, or if WSD cancels for non-payment, you will owe either the unpaid balance of the remaining plan price or the full retail price of used benefits minus any plan payments made. WSD may charge this amount using your nominated payment method.

11. Limits on Use of Discount: The plan discount cannot be combined with other offers or promotions, including Southern Cross discounts, corporate offers, dental exam and X-ray offers, new patient offers, or GoldCard discounts. It is also not applicable with government-funded programs such as Ministry of Health Dental Benefits, WINZ, or ACC payments. The discount cannot be used with Q-card finance offers or Afterpay.

12. Data Retention and Deletion: WSD retains personal information as required for providing and renewing the plan, pursuing legitimate business interests, audits, legal compliance, dispute resolution, and enforcing agreements. Information will be deleted upon request unless WSD is legally required to retain it.

13. Additional Notes: The plan is non-transferable and cannot be redeemed for cash. Benefits cannot be paused or deferred and do not roll over. Patients are responsible for notifying participating practices of changes to contact details. Appointment changes must comply with the practice cancellation policy. Failure to cancel with sufficient notice may result in the treatment being considered redeemed. If your provider is unavailable, another qualified provider may be assigned.

14. Excluded Treatments: Excluded areas include orthodontic treatment such as fixed braces, removable braces, clear aligner therapy including SureSmile, functional appliances, space maintainers and orthodontic retainers. Dental implants and associated treatment are also excluded, including surgical placement, bone grafting, intravenous sedation, implant-supported crowns and bridges, and implant-supported dentures, as well as partial and full dentures where excluded from discount eligibility.
